There are many reasons why it may be helpful to consult a genetic counselor prior to becoming pregnantBelow are several more common examples of testing and indications for scheduling a genetic counseling appointmentHowever, this list is not exhaustive, and each patient may have a unique question to address.  

Preimplantation Genetic Testing (PGT)

Preimplantation genetic testing (PGT) is a form of genetic screening used through the process of vitro fertilization (IVF). PGT can be used to screen embryos for chromosomal abnormalities or identified at-risk genetic conditionsThe goal of PGT is utilize the genetic test result to aid in embryo transfer decision-making. 

There are various forms of PGT: 

  • PGT-A: screening for Aneuploidy, or differences in the total number of chromosomes 
    • Available to any IVF patient/couple 
    • Screens for egg age-related chromosome differences  
  • PGT-M: Screening for a Monogenetic (a single gene condition) 
    • Only a subset of IVF patients are eligible 
    • Either a patient or partner (or both) need to carry the genetic condition which embryos are at-risk to inherit 
  • PGT-SR: Screening for Structural Rearrangements 
    • Patients with identified chromosomal balanced translocations or inversions  
    • Decreases the risk of a pregnancy with an unbalanced rearrangement which may result in miscarriage or a baby with special health needs.

Some patients present to the Reproductive Medicine Center due to a history of pregnancy lossIn some cases, meeting with a reproductive endocrinologist can assist with identifying an explanationSome patients may be referred for genetic counselingAt a genetic counseling appointment patients can expect a thorough family and personal medical history to be taken to assess for potential genetic or hereditary causes which can impact reproductive risksGenetic testing may be offered through this process such as a chromosome analysis and/or additional genetic testing.
