Complex Gynecology

A chronic inflammatory condition where endometrial-type cells exist outside of the uterus. Patients can experience painful periods, painful intercourse, and infertility. It can also lead to internal scarring and adhesions of pelvic organs.

Pelvic pain can arise from a variety of causes, both gynecologic and non-gynecologic. Proper diagnosis and management requires a careful review of symptoms, examination, imaging, and prior treatments. Our team specializes in navigating this condition.

Benign muscular tumors of the uterine wall. These can cause abnormal bleeding such as prolonged, heavy, or frequent menses, or large bulk symptoms of abdominal fullness, urinary urgency, or constipation.

Any level of bleeding concerning to the patient warrants evaluation. Abnormalities may be within timing of bleeding, heaviness of flow, duration of bleeding, and frequency of menses.

A condition similar to endometriosis, but in which the endometrial-like cells exist within the wall of the uterus. The disease is highlighted by painful menses and abnormal uterine bleeding, and can potentially lead to infertility as well.

Polyps are small growths within the cavity of the uterus that can cause abnormal uterine bleeding. Most often these are benign, non-cancerous, and can be removed with hysteroscopy.

It is normal for an ovary to ovulate, or develop a cyst (better called a follicle in this situation) every month. But sometimes, other conditions can lead to development of ovarian cysts. Common ovarian cysts include simple cysts, endometrioma, dermoid cyst (teratoma), hemorrhagic (bleeding) cysts, and fibromas. Not all cysts require surgery. If surgery is necessary, personalized discussion about ovarian preservation via cystectomy or complete ovarian removal is necessary.

Cervical dysplasia is a precursor to cervical cancer. This is detected through routine cervical cancer screening with Pap smears and human papilloma virus (HPV) testing. Abnormal results may require colposcopy with biopsy, and even cervical conization with LEEP procedures to help remove abnormal cells prior to cancer development.

Menopause is typically diagnosed by lack of menstrual bleeding for at least 12 months (as long as no hormonal medications being used). Any vaginal bleeding after this deserves proper evaluation, as this is the most common presenting symptom of endometrial cancer. Most often, the cause of the bleeding is not cancerous though.

A variety of conditions can cause irritation of the vulva and vaginal. A careful exam and review of symptoms can best determine treatment options. Biopsy is sometimes necessary.
